Certificate of Violation

At least once every 12 months, every CMV driver must provide a list of moving violation they had during that time period. This list of violations is included in 391.11(b)(6) as a minimum requirement to be a qualified driver.  Annually this is one step of a three step process: 1) The driver signs his/her Certificate of Violations, 2) the motor carrrier orders the driver’s motor vehicle report (MVR) to verify violations, and 3) then the motor carrier certifies that the driver still meets the minimum requirements to be a driver on the annual review form.
